Evidence Form (WCA6)
The Evidence Form (WCA6) will be submitted to Cumberland Council as supporting evidence for our Definitive Map Modification Order relating to the Right of Way application.
If you would like a map to accompany your form, you can request one below. You may use either of the maps provided or submit your own.
Other Forms
- Print and complete the Questionnaire.
- Print and complete the Membership Form.
- If needed, complete the Standing Order Form to present to your bank.
Once completed, you can return your documents by email or drop them off at The Hub, Hallbankgate, or Halton Lea Gate Post Office.

If you are completing a Right of Way Evidence Form (WCA6) and need a map, you can request one by using the contact form below.
By requesting a map, you confirm that:
- the map is for your sole use;
- it will be used only in connection with your WCA6 Evidence Form submission; and
- you will not distribute it in any other way.
You will receive a printable map automatically in response to your request.
